Family Practice/Primary Care Physician Assistant

Nurse Practitioner / Physician Assistant - Cape Cod, MA
Type: Full-Time or Part-Time
Setting: Skilled Nursing Facilities & Assisted Living Facilities
Work Location: In-person

Responsibilities

• Provide primary care visits 3-4 days per week
• Perform medication reconciliations and annual wellness visits
• Examine patients and review medical records
• Order, review, and interpret diagnostic tests (labs, X-rays, etc.)
• Diagnose medical conditions and illnesses
• Develop treatment plans for chronic and acute conditions
• Prescribe appropriate medications
• Maintain accurate and timely records

Qualifications

• Valid Nurse Practitioner (NP) or Physician Assistant (PA) license in the state of MA
• Graduation from an accredited NP or PA program
• New graduates are welcome -especially those with clinical rotations or experience in primary care, internal medicine, geriatrics, or skilled nursing settings
• Strong assessment, diagnostic, and clinical decision-making skills
• Ability to work independently in a round-and-go model while collaborating with a multidisciplinary team
• Excellent communication and patient-education skills
