We are seeking experienced
Chart Abstraction Analysts
to join an established quality review team supporting clinical measure compliance and care gap closure initiatives. This role focuses on reviewing electronic medical records to validate patient compliance with quality measures, identify exclusions, and ensure care delivered both internally and externally is accurately captured for reporting purposes.
This is a
fully remote, long-term contract
opportunity requiring consistent availability during
Central Standard Time (CST) hours .
What You’ll Be Doing Review patient charts in
Epic
and
Care Everywhere (CE)
to determine compliance with assigned clinical quality measures and HEDIS requirements. Identify missing care gaps and execute
closed-loop processes , including creating encounters to capture care completed outside the primary health system. Validate patient eligibility and determine appropriate exclusions based on project specifications. Track findings, closed gaps, and productivity using
Excel spreadsheets
and secure
SharePoint
drives. Submit and manage medical record requests via
RightFax
when documentation is missing. Coordinate with internal teams to support ordering, follow-up, or outreach tied to quality measures. Participate in weekly touch-base meetings and adapt to instruction updates as measures rotate. Required Qualifications Experience with
chart abstraction, HEDIS, or clinical quality review . Strong working knowledge of
Epic , including
Care Everywhere (CE) . High proficiency with
Microsoft Excel
for tracking and reporting. Ability to manage a defined patient list independently while meeting monthly productivity expectations. Comfortable working
CST business hours . Strong attention to detail and ability to interpret complex medical documentation. Preferred Background Prior experience as a HEDIS Abstractor, Quality Analyst, or Chart Reviewer. Clinical background (RN, LPN, MA) or experienced non-licensed abstractors with strong Epic exposure. Experience rotating across multiple measures or quality projects.
