LVN Discharge Planner at Providence Little Company of Mary Torrance in Torrance, CA. This position is Per Diem and will work 8-hour, Day shifts. Collaborates with the patient/family, the acute multidisciplinary team, and post-acute providers to implement the patient’s discharge plan. Proactively escalates risks and barriers to discharge to the Case Manager. Documents all discharge planning efforts in the patient’s medical record in a timely manner. Maintains ongoing communication with the multidisciplinary team and follows all regulatory requirements and ministry policies regarding care coordination/discharge planning. Required Qualifications

Upon hire: California Licensed Vocational Nurse. 1 year Healthcare related position. Preferred Qualifications

Associate’s Degree Healthcare related. Benefits

Base pay $34.70 - $53.86; additional compensation may be available for this role, such as shift differentials, standby/on‑call, overtime, premiums, extra shift incentives, or bonus opportunities. EEO Statement
