Job Details:

• M-F 8-5
• Complex care, Incoming patients are invite-only
• Seeing patients 16 and up, 8-10 patients per day
• The phone call is shared 1 in 5
• Join 4 other physicians
• EMR is E clinical works
• onsite infusion center
• BLS certification and a strong focus on customer service is required
• Experience in population health management and working with psychosocially and medically complex patients is strongly preferred
• collaborate with a team that includes a nurse practitioner, medical assistant, nurse care manager, clinical social worker, and office coordinator
