Job Title: Case Manager Registered Nurse (RN) Location: Apple Valley, CA 92307 Shift: Day Shift (5x8-hour, 08:00 AM - 04:30 PM) Payment Breakdown: $23/ hr. on W2 ($920 weekly) + $50/hr. as Stipend ($2,000 weekly max stipend) Key Responsibilities: • Assess patients and coordinate care. • Organize and participate in family meetings. • Develop and participate in treatment plans. • Work with HMOs, IPAs, and other healthcare networks. • Arrange DME, SNF, and ambulance transport. • Support patients with mental health issues, homelessness, and substance abuse. Requirements: • 1 year of acute care case management experience. • Experience in care coordination, discharge planning, and utilization review. • Competency in arranging DME and transportation independently.
