Key Responsibilities
Record and manage information on inquiries, admissions, and discharges.
Coordinate and facilitate the admissions process, including completing necessary paperwork.
Communicate admission updates to facility representatives.
Maintain accurate monthly census records in collaboration with the Business Office.
Provide regular reports on referral leads, admissions, and discharges.
Work closely with Social Services, Nursing, and the Business Office on bed availability.
Evaluate referrals for admission candidacy, including home or community agency visits as needed.
Maintain relationships with referral sources and notify hospitals of bed availability.
Qualifications
High school diploma or equivalent.
Experience in admissions, preferably in long-term care (LTC) or skilled nursing facility (SNF).
Positive, energetic, and creative attitude.
Ability to work independently or collaboratively.
Proficient with computers.
