Purpose The Jewish Board’s Community Behavioral Health treatment programs provide compassionate, high quality, evidence-based services to individuals and families in the communities we serve. Our staff use a culturally competent, person-centered approach to help individuals and their families develop skills and resources to improve overall functioning, to instill hope, and to strengthen resiliency. We work closely with community partners to address health disparities while celebrating the strengths and resilience of our communities.
Program Description The Substance Use Disorder (SUD) Program is an Article 32 OASAS licensed outpatient program providing culturally affirming, evidence-based counseling and medication-assisted treatment to adolescents and adults with SUD diagnoses and their families. Treatment approaches are trauma-informed and individualized. The program is part of the Midwood Brooklyn Hub, a certified Community Behavioral Health Center that serves all community members. It offers evening and weekend appointments to provide optimal access to care.
Position Overview The Substance Abuse Counselor uses a trauma-informed, person-centered approach in the evaluation, treatment, and care coordination of individuals with substance use disorders and their family members. The counselor provides clinical care as part of an interdisciplinary team and collaborates with clients and other treatment providers in developing treatment plans and helping clients work toward their goals.
Key Essential Functions
Conduct comprehensive psychosocial evaluations of clients seeking substance use disorder treatment and incorporate feedback from families and external sources.
In collaboration with the client, family, and treatment team, develop treatment plans to help clients achieve goals across multiple life domains.
Provide individual and group counseling for clients and their family members using trauma-informed, evidence-based interventions.
Provide care coordination to address identified needs and help clients access services to support attainment of treatment plan goals.
Develop discharge and aftercare plans to ensure clients are connected to the appropriate level of care and support treatment gains.
Monitor response to treatment interventions through urine toxicology screens, breathalyzers, and other screening tools to inform the treatment planning process.
Accurately and timely complete all documentation.
Provide psychoeducation and training on substance use disorders to other Jewish Board programs and community groups.
Other duties as assigned.
Educational / Training Required New York State Credential in Alcohol and Substance Abuse Counseling (CASAC) required; Bachelor’s Degree preferred.
Experience Required / Language Preference
Fluency in Spanish desirable.
Computer Skills Required
Experience with documenting in electronic health records; comfortable with technology, including use of email, Microsoft Word Office software (Word, Excel) and video conferencing platforms such as Zoom.
Visual and Manual Dexterity
Ability to operate computer devices (mouse, keyboard).
Work Environment / Physical Effort
Work will be conducted both on site at the office and off-site at various community locations as needed to serve clients.
