Pay range:
$25–28/hr (depending on experience)
Schedule/Shift:
Full‑Time, Monday–Friday, Day Shift
Responsibilities
Retrieve referrals in a timely manner from Forcura, ECIN, or other submission methods.
Initiate data entry and start insurance authorization process immediately.
Arrange and schedule transportation (as needed), Durable Medical Equipment (DME), and medication delivery.
Contact families and patients to coordinate Start of Care or initiation visits, and delivery of medications and DME.
Schedule initiation visits and subsequent care visits in HCHB and send visit to RN/LVN device for care initiation.
Qualifications
High school graduate.
Valid state Medical Assistant (MA) certification.
Minimum of 2 years of healthcare experience.
Knowledge of hospice purpose and function, and acceptable diagnoses for hospice settings.
Proficiency in word processing, desktop publishing, and spreadsheet software.
