Compensation Range : $65,000 - $70,000 yearly commensurate with experience.

Overview Using a one-to-one and group person-centered counseling model, Community Oriented Recovery and Empowerment (CORE) program seeks to improve the functioning of individuals living with mental health diagnoses, histories of chemical and alcohol dependence, or physical disabilities, helping them achieve personal goals in the areas of recovery, employment or independent functioning.

Responsibilities

Community Psychiatric Support and Treatment (CPST) Services: Provide time-limited, goal-directed interventions included in the ISP.

Conduct person-centered assessment, service planning, documentation, and collaboration; provide individual and family psychotherapy and counseling and integrated co-occurring disorder treatment.

Actively engage around crisis prevention; offer customized interventions for intensive rehabilitation counseling.

Psychosocial Rehabilitation (PSR) Services: Engage in person-centered planning process and support skill building, psychoeducation, and development of personal coping strategies.

Provide rehabilitation counseling to improve functioning in living, working, learning, and social environments.

Maintain a caseload of clients providing approximately 25 hours of clinical care weekly.

Assist clients in articulating personal goals using a client-centered approach.

Work independently and as part of a team, collaborating with professionals across disciplines.

Conduct intake assessment, counseling, advocacy, and referral services to HARP members.

Receive training in evidence-based modalities.

Hybrid work requirement with both remote and in-person formats, majority of work in the community with clients.

Provide services as outlined in each participant’s Plan of Care.

Assist clients with referrals to necessary services and agencies.

Collaborate with clients’ care teams to determine steps needed for self-directed recovery.

Teach clients to identify and combat negative self-talk and overcome fears.

Support vocational/educational choices and assist with job selection, anxiety, and interview tips.

Maintain case records, progress notes, and individualized service plans in compliance with state regulatory requirements.

Hold case conferences as needed.

Perform other duties as assigned.

Minimum Requirements

Master’s degree in the Healing Arts with an active license in one of the following: LMSW, LMFT, LMHC, or LCAT.

Experience serving people with mental health diagnoses or substance use preferred.

Seniority level: Associate. Employment type: Full-time.
