full-time, board certified or board eligible Family Medicine Physician
to join our dedicated team.
Practice at two locations: our main clinic and a satellite office at Pleasant View Retirement Community Team includes 6 Physicians and 5 Advanced Practice Providers Collaborative model with nursing, care coordination, and referral support Meaningful work with a diverse patient population, including residents of a senior living community
Core Responsibilities Candidates must be able (with or without reasonable accommodation) to:
Consult with patients to determine diagnosis and care plan Order and interpret tests to support clinical decisions Monitor lab results, ensure medication safety and adherence Administer or prescribe medication, vaccines, and therapeutic interventions Clearly explain procedures, results, and care plans to patients Work closely with a multidisciplinary team including nurses, social workers, therapists, and pharmacists Refer patients to specialists as needed
Minimum Qualifications Medical Degree (MD or DO) from an accredited medical school Board Certified or Board Eligible in Family Medicine Completion of an ACGME-accredited Family Medicine Residency
