Hope For Youth is a nonprofit organization specializing in child welfare services. For over 50 years, Hope For Youth strives to provide positive outcomes for children, adolescents, and families. Program Overview The Nassau Prevention Program ("H.E.A.R.T" – Helping Enhance Attachments And Remain Together) is a time‑limited, family‑oriented, supportive and rehabilitative child welfare program that:
Averts a disruption of a family that could result in imminent placement of a child in foster care.
Enables a child placed in foster care to return to his/her family earlier than otherwise possible.
Reduces the likelihood that a child discharged from foster care would return to such care.
Responsibilities
Function within the policies and procedures outlined in the program practice guide and employee manual.
Administer the program, including planning, organizing, launching, and ensuring its continued success.
Collaborate with clinical supervisors to coordinate the delivery of clinical services.
Develop and implement policies and procedures for ongoing success in accordance with HFY practices.
Meet weekly with the Program Supervisor.
Communicate as needed with supervisory personnel at Nassau DSS and the court.
Gather program statistics to prepare reports for submission.
Participate in treatment team, case review, and other meetings.
Work collaboratively with referral resources and other agencies serving families and the court.
Act as part of an interdisciplinary team.
Provide support for other projects and locations; travel as needed.
Sit in on agency committees such as Training and Staff Development and Continuous Quality Improvement.
Adhere to state regulations and applicable code of conduct.
Perform other work necessary to meet the needs of clients, the program, HFY, oversight agency, and regulatory obligations.
Qualifications
LMSW with at least 7 years of supervisory experience.
Knowledge of and experience working within child‑serving systems.
Strong communication and leadership skills.
Excellent written, verbal, and computer skills.
Flexible work schedule to accommodate program needs.
Valid NYS Driver’s License in good standing.
Physical ability to maintain reliable vehicle use, attend home visits, work from various HFY locations, and perform manual tasks such as walking, standing, bending, kneeling, squatting, climbing stairs, reaching overhead, repetitive wrist/finger movements, and lifting up to 50 lbs.
Compensation Salary: $74,000 – $75,000 per year.
