Admissions Care Partner

to join our team in

Fort Lauderdale . This position is

onsite and full-time.

Working in our

affiliated hospitals

throughout

Broward County , this role follows a

Thursday through Sunday schedule, 9:00 a.m. - 7:30 p.m ., helping ensure patients and families receive a smooth, supportive experience during an important stage of care.

Trustbridge Hospice

has cared for more than 200,000 South Florida families. As a community-based nonprofit, we provide 24/7 hospice and palliative care, along with caregiver support and bereavement services for families facing serious illness.

What You’ll Do Present our hospice program and continuum of care in a positive manner to the families/patients and facilitate timely admissions. Process pending referrals, pending list phone calls, obtain signed consents, and complete non-nursing documentation in a healthcare setting. Assisting with the admissions process from Intake to the Field and responding to patient needs by representing the Trustbridge values. Functions as a problem solver and facilitator with the principle objective of providing continuity of care to meet the needs of patients and families. Assists with coordinating the plan of care and treatment (in person or telephone) for hospice appropriate patients and their families. Able to articulate all components of the organization's continuum of care, guiding patients and families to necessary services.

What You’ll Need Bachelor's Degree preferred (in health related or human services field). 3 to 5 years of experience in customer service/referral representative in healthcare and/or community settings. Strong understanding of medical terminology preferred. Hospice experience highly desirable. Must possess ability to communicate physical and psychosocial needs of patients and families. Organized, flexible, self-motivated with the ability to communicate to all levels of a professional medical team. Able to work independently with minimum supervision. Works effectively under stress and under deadlines.
