Program Coordinator - Outpatient Services
Join Fremont Hospital - Behavioral Health as a full‑time Program Coordinator to support and strengthen our outpatient programs.
This role is ideal for someone who is a self‑starter, naturally organized, and enjoys juggling moving pieces. The Program Coordinator plays a central role in supporting clinical operations within acute outpatient services while ensuring the smooth day‑to‑day functioning of the program.
The Program Coordinator serves as the primary point of contact for patients entering outpatient services. Responsibilities include scheduling and coordinating all patient assessments, explaining available services and levels of care, guiding patients and families through next steps, and coordinating appointments within the program. This role works closely with clinical and administrative leadership to support care coordination and project‑based initiatives across Partial Hospitalization (PHP), Intensive Outpatient (IOP), and inpatient levels of care. The Program Coordinator also tracks attendance and engagement, helping ensure accurate communication and continuity of care.
In addition to care coordination, the Program Coordinator supports the operational infrastructure of the outpatient program. This includes administrative and programmatic tasks such as creating and maintaining treatment planning tools, managing attendance tracking and incentive‑based initiatives, supporting scheduling workflows, and assisting with program improvement projects that enhance efficiency and patient experience.
The Program Coordinator also functions in an office management and patient‑facing capacity, serving as one of the first points of contact for patients and families. Responsibilities include covering the front desk, welcoming and checking in patients, managing incoming calls, maintaining supplies, and ensuring the outpatient office runs smoothly and professionally. The role also includes helping plan and support patient‑centered events and activities, and is ideal for those that love to host and create experiences for others.
This position works under the guidance of the Executive Director of Business Development, reports directly to the Administrative Director of Outpatient Services, and collaborates closely with the Program Manager and multidisciplinary treatment team to support both daily operations and longer‑term program goals.

Education
High school graduate or equivalent; college or university experience preferred.
Experience
At least two (2) years of related experience in a human service, social service, psychiatric, or mental health setting, performing triage for inquiries and providing excellent customer service.
Knowledge of psychiatric patient care techniques with understanding of mental illness.
Experience in facilitating patient care, program implementation; knowledge of psychotherapeutic interventions and psychiatric diagnoses.
Certification(s)
CPR with AED (CPR/AED) certification must be current and a copy of your certification must be presented to Human Resources before hire.
You must successfully complete Fremont Hospital’s Non‑Violent Crisis Intervention (CPI) training course (Blocking/Disengagement Skills Only) within 90 days of employment and prior to assisting with preventative intervention measures.
