This role offers a base pay range of

$71,421.00/yr - $71,421.00/yr . The actual pay will be based on skills and experience.

Purpose of Position The Addiction Counselor serves as an integral member of the multidisciplinary team, developing and implementing clinically sound, culturally relevant treatment programs, case management, care coordination, and other services for patients in Substance Use Disorder (SUD) treatment. The role promotes recovery and self‑sufficiency and ensures compliance with regulations and NYC Health + Hospitals standards.

Essential Duties and Responsibilities

Conduct initial screenings, assessments, and ongoing toxicology screenings; develop psycho‑social histories; formulate diagnostic impressions; prepare accurate screening, intake, and assessment reports.

Facilitate individual, group, and family counseling; provide psycho‑education and skills training.

Make decisions and maintain required records regarding appropriate Level of Care using patient criteria tools.

Develop and monitor treatment/recovery plans; initiate discharge planning; accurately record treatment plans and outcomes.

Participate in evaluation of treatment progress; maintain confidential case records and files; document progress in timely and accurate manner.

Discuss financial responsibility with patients; provide money management assistance.

Provide orientation to new patients and families; establish relationships; offer support and referrals.

Coordinate and provide case and care management services integrating supportive resources.

Provide crisis intervention and support; seek additional assistance when needed.

Advise staff on patient needs for job training, housing, education; make field visits.

Conduct outreach activities and advocacy to educate staff and communities on substance abuse.

Participate in QA/PI activities, trainings, and mandated health care events.

Maintain required certification(s) and completing continuing education; comply with policies and regulations.

Perform other related duties as directed.

Minimum Qualifications

Proof of completion of OASAS “Supporting Recovery with Medications for Addiction Treatment (MAT)” training.

For Level II assignment:

Valid CASAC – Level 1 Certificate issued by OASAS and one (1) year of full‑time, progressively responsible experience in SUD assessment, treatment planning, counseling, case and care management.

Otherwise considered a QHP, with one (1) year of full‑time experience.

Equivalent education, training, and experience, with a CASAC Level 1 Certificate or meeting QHP definition.
