TAG MedStaffing is hiring a Social Worker (MSW) in Brooklyn, NY for our client, supporting assessments, safety planning, case management, and counseling for older adults experiencing abuse or neglect. This community-based role includes conducting psychosocial assessments, coordinating benefits, collaborating with community partners, and providing ongoing counseling and crisis intervention. Ideal candidates have experience in aging services, elder abuse, or community social work.

Schedule Monday to Friday, 9:00 AM – 5:00 PM

Responsibilities as the Social Worker (MSW)

Clinical Assessment & Safety Planning:

Conduct in-home and in-office assessments, develop case plans, and create individualized safety plans using evidence-based tools.

Counseling & Case Management:

Provide individual and family counseling, monitor safety, conduct reassessments, and update care plans as client needs evolve.

Benefits & Service Coordination:

Screen eligibility, assist with applications, and coordinate services including transportation, home care, legal aid, psychiatric evaluations, and housing support.

Financial Advocacy & Community Collaboration:

Assess financial risk, support financial management needs, collaborate with community agencies, and participate in E-MDT meetings and outreach.

Documentation & Crisis Response:

Maintain accurate electronic case records and reports, participate in supervision and training, and escalate crises or high-risk situations as needed.

Qualifications for the Social Worker (MSW)

Education:

Master of Social Work (MSW) degree from an accredited institution required.

Experience:

1+ year of social work, case management, elder abuse services, behavioral health, or community-based clinical experience.

Technical Skills:

Strong written communication, documentation, and computer skills with the ability to maintain electronic case records.

Industry Knowledge:

Understanding of aging services, trauma-informed care, benefits/entitlements, and New York social service resources.

Skills & Attributes:

Reliable, empathetic, highly organized professional with strong interpersonal skills and the ability to work independently in the field.

Other:

Bilingual skills preferred.
