Division Description Felton Children, Youth, Family, and Transitional Age Youth division offers comprehensive, early care and education, family support, and mental health services across San Francisco to over 400 families. Via a two-generation approach, Felton’s Family Support services promote strong families and optimal development for their children by focusing on the Strengthening Families Framework. The primary function of the family supports The Five Protective Factors; parental resilience, increase social connections, increase knowledge of parenting and child development, providing concrete support in times of need, and supporting social‑emotional competence in young children. This approach serves as a one‑stop integrated service continuum, supporting families to address their needs largely under one roof, with a core group of staff with whom they can build trusting, supportive relationships.

This approach serves as a one‑stop integrated service continuum, supporting families to address their needs largely under one roof, with a core group of staff with whom they can build trusting, supportive relationships.
The FRCs at Felton Institute consist of two comprehensive Family Resource Centers (FRC) with a multitude of wraparound services to serve the young parent population citywide and families enrolled in the early care and education programs. Our FRCs are inclusive of all cultural backgrounds and family compositions. Key programming includes: Intensive care management, support groups that encourage parent leadership and healthy family development, Workshops and classes focused on child and youth development, health and wellness, Family Engagement, Community events, Curriculum‑based parenting workshops (STEP, PIPE, Abriendo Puertas—a Latino‑focused curriculum centered on family strengthening), and parent leadership/ advocacy, mental health support and counseling, family economic success, fatherhood programming, GED preparation, post‑secondary education, support applying to programs such as Cal‑fresh, Food Stamps, and Below Market Rate housing.
Job Description The Bilingual Case Manager is responsible for working with families enrolled in our Young Family Resource Center (YFRC). The Case Manager provides family advocacy, resource and referral and ensures that his/her assigned clients have a coordinated range of client‑centered services linking them with health care, educational and social services. The Bilingual Case Manager is responsible for completing initial and bi‑annual assessments with clients to include completion of a care plan in addition to seeing clients periodically to address their current needs and checking on progress. The Bilingual Case Manager is responsible for entering all client data, units of service, and case notes into the database in a timely manner. As assigned, the case manager will participate in family support services network meetings and support family support program activities. The Bilingual Case Manager reports to the Family Support Supervisor.
Program Specific Responsibilities
Carries a caseload of up to 50 ECE families or young pregnant or parenting teens as assigned.
Documents service delivery in our Salesforce Data Base and develops hard files to comply with our funder requirements and quality assurance.
Conduct initial comprehensive assessment of clients’ needs and support systems.
Develop a comprehensive, individualized service plan and coordination of the services required for the implementation of plans for clients.
Establishes individual service plans and routinely monitors service delivery; updates the care plan and reassesses service needs on quarterly basis.
Provides intensive and wrap‑around case management including referral and support services with a form intake, needs assessment and a facilitated planning process to assist families to develop a plan‑of‑action to meet their individual, family, and community goals.
Assessing clients’ physical and mental wellness, needs, preferences and abilities and using these to develop tailored care plans. Connect with members of the multidisciplinary team to coordinate services.
Help families receive adequate and convenient services to meet their needs and have access to only public benefits.
Listens to clients’ concerns and providing counseling or intervention, resource, and referral information as needed.
Maintains contact with client and/or client’s support system through office, telephone, field, or home visits and by way of minimum monthly face‑to‑face contacts and quarterly home visits.
Records clients’ progress including charting referrals, home visits and other notable interactions.
Evaluates clients’ progress periodically and adjusting as needed to improve outcomes.
Conduct regular check‑ins and home visits (including virtual visits).
Follow up with clients to ensure they were satisfied with services.
Maintains clients’ records by reviewing case notes; logging events and progress.
Provides support to Family Support team in case management, mental health services and crisis intervention for clients.
Conducts outreach internally and externally via presentations, flyers, and social media.
Adheres to Federal Financial Participation (FFP) requirements that include completion of time study reports, and secondary documentation identifying activities completed during the time study period along with outlook and client case notes that correspond to the activity.
Ensure program compliance with MCAH & First 5 contracts and support for maintaining client’s files.
Represents Family Support Team in various meetings with Community Base partners in outreach and community partnership networks.
Additional duties as needed.
Minimum Qualifications
A minimum of 3 years of case management or treatment of high‑need individual or families.
Bachelor’s Degree in Social Work or related field.
Compassionate and able to relate to different clients with various needs.
Motivational to encourage clients to follow their care plans.
Strong verbal and written communication skills to explain to clients, family members and friends and professionals the case and care plan and maintain good case records.
Critical thinking and problem solving to determine the best care plan for each client after assessing clients, analyzing notes from health care and social workers.
Flexibility to change care plans if they are not getting the best results.
Organization to manage several different cases at once.
Computer literacy to maintain and manage case records.
Working knowledge of Microsoft Word processing (i.e. Microsoft Word, Excel and Access) Ability to work with and maintain confidential information.
Ability to be a team player with a welcoming attitude.
Bi‑lingual in Spanish/English REQUIRED.
