Under the direction of the CARE Program Director, the CARE Transition Coordinator supports youth transitioning to and from the Buchanan YMCA’s Center for Academic Re-Entry and Empowerment (C.A.R.E.) program into comprehensive or credit recovery high schools and post-secondary opportunities. This role provides case management, training, workshops, and transition planning while building strong partnerships with SFUSD schools, community-based organizations, and other stakeholders to ensure student success. Key Responsibilities

1. Student Support & Case Management Provide individualized transition planning and case management for students. Support students in creating and achieving Individual Learning Plan goals. Mentor and coach high school-aged youth, including those requiring intensive emotional and behavioral support. Advocate for students through case consultations, CFTs, and IEPs. Facilitate life skills/advisory and enrichment workshops. 2. Communication & Relationship Building Maintain positive relationships with school administrators, teachers, parents, and community partners. Communicate student progress to parents/guardians and respond promptly to concerns. Complete monthly reports and audits for DCYF and internal programs. Manage alumni database and compile student data. 3. Program Coordination Organize school tours, alumni events, and family/community engagement activities. Co-lead field trips and enrichment activities. Assist with short- and long-term planning for Buchanan Alternative Education Programs. 4. Safety & Behavior Support Uphold YMCA and SFUSD safety policies, including mandated reporting. Assist teachers with behavior management and conflict mediation using restorative practices. Facilitate restorative circles in collaboration with student support teams. Qualifications

Bachelor’s degree in social work or related field, or equivalent experience Experience working with youth facing barriers to academic success Strong communication and relationship-building skills Ability to work Monday–Friday during program hours, with occasional evenings/weekends CPR & First Aid certification (or ability to obtain upon hire) Experience in strategic planning and community collaboration Case management credential Familiarity with local resources and community organizations is helpful but not required Work Environment & Physical Demands

Primarily onsite at Buchanan YMCA with occasional travel to schools and community partner locations. Fast-paced, collaborative environment requiring flexibility and adaptability. Frequent interaction with youth, families, and community partners. Physical Demands

Ability to sit, stand, and walk for extended periods. Occasional lifting up to 25 lbs (e.g., event materials). Comfortable facilitating workshops and engaging in active youth activities. Must be able to respond quickly in emergency situations. Disclaimers

The Transition Coordinator position offers a competitive rate of $25.00 to $30.00 per hour, based on qualifications and experience and is aligned with current salary benchmarking standards.
