Rehabilitation Hospital of Southern California, located in Rancho Mirage, CA, is a premier inpatient rehab facility serving patients throughout the Coachella Valley and Southern California region. We provide intensive rehabilitative care for individuals recovering from strokes, brain and spinal cord injuries, orthopedic surgeries, and other serious conditions.

What you bring:

PT, OT, or SLP preferred; RN. Rehabilitation or interdisciplinary team experience a plus

Ability to harvest, synthesize/analyze data to develop and implement best practice

Ability to build strong relationships in interdisciplinary and patient space

Passionate patient advocate assisting in the referral, admission, to discharge process.

Strong clinical skills with a mindset for process improvement

Qualifications Required Skills:

Clinical license required (PT, OT, SLP, or RN) in the state of employment required. Other licensure/certification to be considered based on appropriate qualifications.

Two years recent experience in acute care setting preferred.

Leadership/supervisory experience required.

Responsibilities The Director of Patient Outcomes (Rehab) is the primary individual responsible for implementing and coordinating the patient assessment and evaluation process for all patients treated by the staff in the Rehabilitation Department. Uses his or her expertise and training in rehabilitation services and independent discretion and judgment to supervise and manage the completion of the Patient Assessment Instrument for each rehabilitation inpatient. Provides oversight and clinical support in completion of data contained in the patient assessment (PAI) and Quality Indicator data for each patient to ensure accuracy in the scoring of functional status and reflection of the patient’s care needs. Direct Case Management Department, as needed. Integrates the hospital’s mission and “Guiding Principles” into daily practice.
