We are seeking a dedicated Nurse Practitioner (NP/PA) for our Primary Care team in Greenwich, CT.
Provide comprehensive, patient-centered primary care services, including health assessments, diagnosis, treatment, and management of acute and chronic conditions.
Collaborate with physicians and healthcare team members to develop and implement personalized care plans.
Educate patients on health promotion, disease prevention, and management strategies.
Maintain accurate documentation and ensure compliance with all regulatory standards.
Participate in quality improvement initiatives and health screenings.
