GHR is proud to represent an exciting Physician Assistant opportunity in Walnut Creek, CA with an established Advanced Care at Home program. This role is part of an innovative joint venture partnership offering cutting-edge hospital-level care in the home setting.

Position Overview:

We are seeking an experienced Physician Assistant to join a dynamic Acute Care at Home team, delivering inpatient-level care to patients safely and effectively in their own homes. This is a full-time, salaried position offering excellent benefits and professional support.

Schedule:

  • 7 days on / 7 days off
  • 8:00 AM 6:00 PM
  • Schedule is fixed and non-negotiable

Location Requirements:

  • Must reside within a 25-mile radius of Walnut Creek, CA

Ideal Candidate Profile:

  • Minimum 2 years of experience as a PA
  • Internal Medicine or inpatient hospital experience required
  • Trauma center experience strongly preferred
  • Comfortable managing acute, complex patients in non-traditional care settings

Candidates that will not be considered:

  • Outpatient-only experience
  • Primary care, urgent care, addiction medicine, or nursing home backgrounds

Compensation and Benefits:

  • Competitive salaried pay structure, paid weekly
  • $1,500 CME allowance per year
  • Reimbursement for ACLS, BLS, and license renewals
  • Mileage and toll reimbursement (with receipts and submitted maps)
  • Full credentialing and administrative support

Key Responsibilities:

  • Serve as the clinical lead in the home for acute care patients
  • Facilitate admissions, assessments, and daily management of patients in coordination with the broader care team
  • Provide patient education and serve as liaison for patients and families
  • Collaborate closely with attending physicians, RNs, and multidisciplinary staff
  • Participate in ongoing quality improvement, safety, and evidence-based protocol development
  • Maintain compliance with infection control, documentation, and safety protocols

Qualifications:

  • Master s degree from an accredited Physician Assistant program
  • Current PA licensure in California
  • Minimum 5 years of PA experience strongly preferred
  • Strong critical thinking, communication, and collaboration skills
  • Ability to work independently in the field with strong support from the remote care team
  • Willingness to adapt in a fast-paced, evolving care model
  • Must pass ongoing federal drug screening
