Annual ER Volume: 2 Documentation System/EMR: EPIC Patient Age Demographic: all What Percentage of Practice is Trauma, Hospital Based, Clinic Based, Surgical, Non-Surgical: 10%

Thyroidectomy, Extended Parotidectomy General Surgical Otolaryngology Requirements:
Tonsillectomy, Adenoidectomy, PE Tube General Non-Surgical Otolaryngology Requirements:
Hearing Evaluation, Eval and Manage Disorders Involving Ears, Nose, Throat and Skeletal Facial Structures Endoscopic Procedures Required:
Septal, Sinus, Bronchoscopy - Flexible, Bronchoscopy - Rigid, Laryngoscopy Facial Surgery Procedures Required:
Rhinoplasty, Facial Fractures, Skin Grafts/Flaps
Facility Reimburse for the Following Costs: Flight, Hotel, Rental Car, Mileage Billed at the IRS Standard Rate
