Position Summary We are seeking an experienced Home Health Occupational Therapist to provide patient-centered care in the home setting. This role is ideal for an OT who values autonomy, meaningful patient relationships, and collaboration with an interdisciplinary care team.
Key Responsibilities
Perform in-home OT evaluations and reassessments
Develop and implement individualized treatment plans
Provide skilled interventions to improve functional independence
Educate patients and caregivers on safety and home programs
Monitor progress and adjust plans as needed
Complete accurate and timely documentation
Collaborate with physicians and care team members
Supervise Certified Occupational Therapy Assistants (COTAs)
Maintain compliance with Medicare, HIPAA, and agency standards
Qualifications
Graduate Of An Accredited OT Program
Active Maryland OT license
Minimum 3 Years Of Clinical Experience (home Health Preferred)
Strong assessment and care planning skills
EMR Experience Preferred (WellSky/Kinnser Or HCHB)
Reliable transportation & valid driver’s license
