Key Responsibilities
Assist patients in navigating service delivery systems and agency procedures
Collaborate with medical case managers and clinic staff to implement patient care plans
Monitor patient progress by tracking attendance and following up on referrals
Help patients overcome barriers by arranging services like transportation or translation
Conduct assessments to evaluate medical and psychosocial needs
Create comprehensive care plans to guide patients in meeting their care goals
Provide information and referrals for medical, psychosocial, and basic needs services
Adapt work methods to accommodate the unique needs of each patient
Document services provided in the EHRS client record within 72 hours
Facilitate access to programs addressing psycho-social needs within Legacy and the community
Advocate for clients to decrease service gaps and remove barriers
Connect with Legacy staff and providers for system navigation support
Collaborate with community-based agencies to enhance social support linkages
Minimum Qualifications
High school diploma or equivalent education
Minimum of 1-year paid work experience with People Living With HIV (PLWH)
Knowledge and familiarity with Ryan White grants and services are strongly preferred
Experience with EPIC systems is an advantage
Spanish fluency or bilingual capabilities are preferred, enhancing patient communication
