The Therapy Department at Autumn Lake Healthcare at Glen Hill is hiring a Physical Therapist PT for a full-time or PRN position at our skilled nursing facility. The role involves evaluating patients, developing individualized treatment plans, motivating patients during treatment, and promoting a healthy lifestyle by improving strength, flexibility, balance, and coordination. We seek a cohesive, collaborative professional who can participate in team decisions and enjoy continuous development.

Qualifications

Active state Physical Therapist PT license is required.

Unlimited employee referral bonus of up to $1,000 each.

Multiple locations offered for PRN coverage; potential full-time opportunity at additional locations.

This is a direct hire position; we are not an agency.

We hire PT candidates who require H‑1B sponsorship (current student visa holders, H‑1B transfer, and new H‑1B filing).

Frequently Asked Questions Am I guaranteed 40 hours? While we cannot guarantee hours, we typically offer a full caseload. Majority of our facilities are in close proximity, allowing you to receive additional hours at nearby locations.

Do you give raises? Staff are eligible for performance reviews, where there is always an opportunity for an increase.

Why is this position open? Therapists/DORs that leave our company usually pursue other practice specialties. While many stay PRN with us, we rarely see our therapists joining a competing SNF.

How does corporate support the therapists on the floors? Majority of our Regional Directors, as well as many corporate members, are therapists themselves. All Directors understand the challenges of Medicare and insurance guidelines and are actively present in facilities, collaborating directly with each team member to ensure a well‑supported work environment.

What type of mentorship or support do you provide for new grads? We offer a comprehensive mentorship program to support new grads from day one. You’ll be paired with an experienced therapist who will guide you through documentation, evaluations, treatment methods, and more. Our approach is tailored to your learning style, and as you gain confidence and begin working independently, your mentor and our clinical team will continue to provide support whenever needed.
