Primary Job Functions

Work in the residence of a person served as personal support and be actively engaged with them in all program activities and benefits — both in the home and in the community.

Provide supervision and training to adults with intellectual disabilities to promote each person’s growth toward their highest potential, following individual plans and goals.

Make sure the house is personalized and individualized to each person’s preferences and personality.

Teach and instruct people served to do as much as possible for themselves, per program plans, avoiding personal cell phone use, reading, TV, writing, etc.

Communicate with day‑program staff about the individual's program needs.

Report to the House Manager or Division Manager concerns related to the person served and house operations.

Utilize approved behavior plans and intervention strategies to enhance progress toward goals.

Act as a positive role model for persons served and Agency staff.

Assist directly with independent living skills: mobility, budgeting, personal hygiene, cleaning, bathing, toileting, nail care, and oral hygiene.

Maintain knowledge of persons served whereabouts at all times.

Attend scheduled meetings with the House Manager and/or Division Manager.

Know routine and emergency medical procedures, including CPR, vital signs, wheelchair usage, transfers, assistive devices, splints, dentures, and walkers.

Administer medication on time and correctly, following the Physician Medical Order Form and Medication Administration Record, ensuring a three‑way check.

Maintain cleanliness and safety of the persons served’s home.

Aid persons served with personal belongings, ensuring items are secure and controlled.

Complete required records and reports: monthly reports, incident reports, seizure records, program activity logs, fire drill reports, etc.

Clean the home, do laundry, and prepare food per approved menus, special diets, and food consistencies.

Collect data on persons served progress per Agency policy.

Transport persons served to appointments, activities, and community outings.

Handle financial responsibilities for Agency and persons served funds per policy.

Communicate medical concerns to the Registered Nurse and supervisor promptly.

Check all rooms at specified times to ensure safety and well‑being.

Use crisis intervention techniques in emergencies when persons served may injure themselves or others.

Know the location of essential house records: fire drills, water temperatures, medical records, menus, and other files.

Attend religious services with the person served.

Use the home computer daily for information and specified job duties.

Participate in in‑service training, staff meetings, and other activities for professional development.

Contribute to a positive, enthusiastic, responsible, and courteous work and home atmosphere.

Work a flexible schedule, including nights, weekends, and holidays.

May be assigned to other residential homes as required by Agency needs.

Treat persons served with kindness, dignity and respect at all times.

Use technology to complete specified job duties.

Attend work regularly and follow the assigned schedule.

Work cooperatively with families, staff, supervisors, administrators, co‑workers, community professionals, customers, neighbors, vendors, and the public.

Abide by Agency policies and procedures.

Assume other duties, responsibilities, and special projects as needed.

Secondary Functions

Attend team meetings as required.

Work cooperatively with Agency volunteers, maximizing their experience while meeting people served and program needs.

Requirements Education:

High School diploma or equivalent preferred; state regulations may vary.

Experience:

Commitment to enhancing the lives of people with intellectual disabilities; ability to work in a team. Must possess a driver’s license and an acceptable driving record per Agency policy. Must be able to lift, pull, and push adequate weight (may exceed 50 pounds).

NOTE:

The Personnel Officer may substitute related experience and/or education in lieu of the above requirements.
