Join our compassionate and dedicated team providing professional care to patients in the comfort of their homes. We are looking for a skilled Home Health Care Registered Nurse who is committed to patient-centered care.
What We Offer
Mileage Reimbursement
$5 differential for visits performed on Sunday and Saturday

The per visit rate for this position is $27
Responsibilities
Delivers skilled nursing care to patients in their homes in accordance with individualized care plans.
Initiates appropriate preventative and rehabilitative nursing procedures, making necessary referrals to community services.
Coordinates services, informing the physician, management, and other personnel of changes in the patient's condition and needs.
Ensures quality and accuracy of patient records in compliance with Medicare and Mass Health standards.
Conducts ongoing medical documentation, writing OASIS assessments as appropriate.
Prepares clinical and progress notes, submitting all required paperwork daily. This is a performance requirement.
Responds promptly to emails and phone calls.
Requirements
Current RN license in the state of Massachusetts
Strong technology skills
Reliable transportation, valid driver's license, auto insurance
