Overview The RN MDS Coordinator is responsible for the completion, accuracy, and submission of the Minimum Data Set (MDS) assessments in compliance with state and federal regulations. This role ensures timely assessments, coordinates care planning, and supports reimbursement accuracy through proper coding and documentation. The RN MDS Coordinator serves as a liaison between nursing, therapy, medical staff, and other departments to ensure comprehensive, resident‑centered care planning.

Responsibilities

Complete and coordinate all MDS assessments and related documentation per CMS guidelines and facility policies.

Ensure accuracy and timeliness of MDS completion, adhering to regulatory deadlines.

Facilitate the development, implementation, and ongoing review of individualized resident care plans.

Participate in resident care conferences as a member of the Interdisciplinary Team as a representative of the nursing department.

Collaborate with interdisciplinary team members to collect assessment data and ensure resident needs are identified and addressed.

Review medical records, progress notes, and therapy documentation to ensure consistency and accuracy of coding.

Educate and support nursing and other staff on care planning, documentation standards, and regulatory compliance.

Advise the Director of Clinical Services as to the status of clinical and therapeutic programming and make recommendations for the development of new programs.

Monitor changes in resident condition to determine if interim assessments are required.

Maintain knowledge of current regulations, coding guidelines, and reimbursement systems related to MDS.

Participate in quality improvement initiatives and audits related to MDS, care planning, and regulatory compliance as required.

Serve as a resource for staff regarding MDS, care planning, and resident assessment processes, keeping abreast changes and providing education to other departments as necessary.

Assist with facility surveys by regulatory agencies as related to MDS and care planning.

Assist in completion of MDS and clinical reimbursement audits.

May perform other varied duties/assignments as directed by the Director of Clinical Services or designee per facility needs.

Qualifications

Current license as a Registered Nurse (RN) in the State of Wisconsin, in good standing.

Prior MDS Coordinator experience strongly preferred; will consider candidates with demonstrated knowledge of MDS and care planning.

Knowledge of CMS regulations, RAI process, and PDPM reimbursement system.

Strong organizational, time management, and attention to detail skills.

Excellent written and verbal communication skills.

Ability to work collaboratively with interdisciplinary team members.

Proficiency in Point Click Care (EHR) systems and MDS software.

Physical Demands

While performing the duties of this job, the employee is regularly required to talk, hear, and see.

Ability to sit for extended periods while completing documentation.

Occasional lifting up to 25 - 50 pounds.

Ability to move throughout the facility to participate in assessments and care planning meetings.

Strong working relationship with members of the management team.

Ability to initiate timely emergency resuscitation measures according to adult resuscitation and advanced life support protocols.

Risk of blood borne pathogen exposure is considered high.
