Intake Coordinator
– Child & Family Support Services, Inc, Pima County.
Job Responsibilities
Conducting intakes and screenings and ensuring all clinical forms are completed and signed.
Coordinating the process of evaluations and assessments to determine initial and ongoing eligibility.
Measuring and tracking progress and outcomes for young people and families.
Attending and participating in weekly team meetings, agency staff meetings and trainings.
Obtaining reports from partner agencies (case managers, referring physicians, hospitals, and treatment facilities).
Coordinating services with community resources, medical and health providers.
Assisting with crisis‑management and supervision when necessary.
Communicating effectively with families, Child and Family Team members (stakeholders, therapists, case managers, etc.), agency management, and Direct Support Specialist Coordinators.
Performing other duties related to the position.
Education and Experience
Related Master's degree and prior behavioral health experience preferred.
Requirements
Must be 21 years or older.
Valid Arizona Driver's License with an acceptable 39‑month driving record.
Reliable transportation, including proof of insurance.
Candidates are required to submit a negative drug test.
Obtain an Arizona DPS Level One Fingerprint Clearance Card.
Complete Adult, Child, and Infant CPR and First Aid training.
