Senior Social Worker – Intensive Case Management

Summary
This position is eligible for the Education Debt Reduction Program (EDRP) – a student loan payment reimbursement program. Candidates must meet specific eligibility requirements and submit an EDRP application within four months of appointment.
Qualifications
United States citizenship.
English language proficiency.
Master’s degree in social work from a CE accredited school.
Licensed or certified by a state to practice social work at the master's degree level.
At least two (2) years of experience post‑advanced practice licensure, with at least one (1) year equivalent to GS‑11.
Preferred experience: prior Mental Health/Homeless program experience.
Responsibilities
Provide clinical and program activities as part of the Mental Health Intensive Case Management program across the North Florida / South Georgia Veterans Health System.
Develop, implement, and assess behaviorally oriented psychosocial rehabilitation programs.
Provide intensive treatment and case management services to assigned Veterans and significant others.
Participate in team staffing and treatment planning, discharge planning, and after‑care coordination.
Maintain clinical documentation per MHICM and Medical Center standards.
Collaborate with other clinical programs, community agencies, and professional organizations as needed.
