Nurse Assessment Coordinator - (RN OR LPN)

Nurse Assessment Coordinator - (RN OR LPN) Join the team at NHC HealthCare Lexington to coordinate assessments for residents in our senior care facilities.

Responsibilities

Organize and schedule assessments while meeting deadlines.

Monitor medical records to validate support of MDS coding.

Collaborate with interdisciplinary team to assure timely completion of Care Area Triggers and Patient Care Plans.

Support accuracy of medical record documentation through ongoing in-services to center partners.

Qualifications

Registered Nurse or Licensed Practical Nurse – graduate of accredited nursing school and currently licensed in the state.

CMAC certification preferred.

Professional experience in geriatrics nursing.

Knowledge and expertise in completing the Resident Assessment Instrument and all applicable rules and regulations.

Knowledge of SNF Part A Medicare and consolidated billing rules.
