Overview StoneGate Senior Living seeks a Traveling MDS Coordinator to support Medicaid and Medicare reimbursement across multiple facilities in DFW and East Texas. The role includes coordinating clinical assessments, supporting documentation, care planning, and ensuring compliance with federal and state regulations.
Responsibilities
Maintain MDS/Care plan schedule for the interdisciplinary team
Coordinate preparation of MDS and Care Plan per regulatory requirements
Track Medicare resident benefit days, validate daily Medicare census, and coordinate with management to ensure accurate billing
Respond to requests for additional information from intermediaries and complete appeals decisions within established time frames
Prepare Medicare denial letters for residents discharged from Medicare coverage within time limits imposed by Medicare guidelines
Participate in ongoing evaluation of documentation required for Medicaid reimbursement
Requirements
Current RN or LVN license in the state of Oklahoma
2+ years of experience in a skilled nursing facility, previously serving as an MDS Coordinator
Proficiency in RAI/PPS processes
Strong attention to detail, documentation, and resident assessment skills
Good communication, clerical, organizational, and computer skills
