MDS Coordinator (RN/LPN)
Outfield Healthcare Partners is looking for an experienced RN or LPN with expertise in the Minimum Data Set (MDS) process to join our team.
What You’ll Do:
Collaborate with the Interdisciplinary Team to assess resident needs and create MDS assessments and care plans in accordance with state and federal guidelines.
Ensure accurate and timely documentation of the MDS process, including coding, medical record reviews, and interdisciplinary updates.
Assist in coordinating daily stand‑up meetings and review care and assessment schedules.
Stay updated on federal, state, and certification guidelines to ensure compliance with all regulations.
Submit resident data to state and federal agencies, and ensure all MDS documentation is accurate and complete.
What We’re Looking For:
Graduate of an approved Registered Nurse (RN) or Licensed Practical Nursing (LPN) program and licensed in the state of practice.
Minimum of 2 years of nursing experience in a Skilled Nursing Facility (preferred).
In‑depth knowledge of the MDS 3.0, Medicare PPS process, Medicaid reimbursement, and OBRA regulations.
Experience with care planning and quality indicators.
Strong attention to detail and excellent communication skills.
