Circle of Life is growing, and we have an opening for an Outreach and Community Engagement Coordinator to join our AZ team. The Outreach and Community Engagement Coordinator will lead community engagement and education efforts within the Navajo Nation, with a focus on high-potential service areas. This role is responsible for building strong, culturally respectful relationships with community members, healthcare providers, and partnering organizations to increase awareness of Medicaid/AHCCCS and ALTCS programs, support eligibility and enrollment, and coordinate referrals to ensure individuals receive timely and appropriate care.

Position Duties Include

Build trust and foster relationships with members of the Navajo Nation and local community organizations.

Conduct outreach activities at community events, provider sites, chapter houses, and other local venues.

Provide culturally sensitive education on Medicaid/AHCCCS and ALTCS programs, eligibility requirements, and enrollment processes.

Deliver in-services, presentations, and distribute informational materials to increase program awareness and access.

Establish and maintain strong partnerships with IHS hospitals, DC planners, social workers, skilled nursing facilities (SNFs), ALTCS case managers, and other referral sources.

Serve as a point of contact for local providers and collaborate to support shared community goals.

Act as a liaison between community members and service providers to ensure smooth, informed transitions into care programs.

Support referral workflows to ensure timely intake, follow-through, and communication with all parties involved.

Track and document outreach activities, events, contacts, and referral data.

Provide regular reports on outreach outcomes, community needs, and trends to support organizational planning and improvement.

Qualifications Qualified applicants should have excellent verbal and written communication skills, excellent interpersonal and customer service skills, organizational skills and attention to detail and time management skills. Industry knowledge is highly preferred.
