Direct services:
Assist SONAR staff in engaging individuals and families who are unsheltered or at risk of homelessness through targeted, purposeful, housing-focused outreach.
Support implementation of aftercare programming for newly housed clients, including check-ins, basic needs coordination, reinforcement of housing goals, and connection to community-based supports.
Provide assistance to SONAR clients in maintaining housing, including supporting recertification processes for housing programs, completion of required forms, gathering documentation, and tracking deadlines.
Provide basic navigation support under the direction of SONAR staff, including assistance with benefits applications, transportation coordination, appointment reminders, and connection to behavioral health, medical, and social services.
Assist SONAR staff in the field as needed, participating in street outreach, encampment engagement, and community-based events to connect individuals to services and housing.
Help maintain and distribute outreach supplies such as water, hygiene packs, basic survival items, resource materials, and forms.
Support event tabling for community education, resource fairs, and landlord engagement activities, including set up, breakdown, information sharing, and collection of contact information or sign-in logs.
Maintain accurate records including services rendered and to whom using the Homeless Management Information System (HMIS) for Maricopa County and proprietary City of Avondale data systems; enters data into system files and databases in a timely and consistent manner; may complete client demographic logs and other statistical information.
Maintain assigned vehicles/trailers (check fluids, tires, wash van regularly, clean inside of van daily.) Maintain supplies (water, hygiene packs, etc) and resources (brochures, paperwork, etc.) in vehicles.
Maintains computerized log of daily activities and completes simple reports and other forms.
Assist with fidelity monitoring tasks, including review of documentation, data quality checks, and tracking of required fields to ensure that SONAR activities align with program expectations, regional standards, and funding requirements.
Attend workgroup meetings, trainings, and participate in continuous quality improvement of the Homeless Services Division.
Answers Department telephones; greets customers and visitors; provides customer assistance; retrieves requested documents; provides information regarding programs, services and procedures; refers callers or visitors to other staff members as appropriate; and takes messages.
Assist with coordination of meetings, trainings, and appointments for SONAR staff, landlords, and partner agencies as requested.
Minimum Qualifications
Associate degree preferred, High School Diploma, GED or equivalency with one (1) year of experience conducting street outreach or case management working with families and/or individuals experiencing homelessness, mental illness, or addiction.
Valid Arizona Operator's Driver's License
Level 1 Fingerprint Clearance Card required within 3 months of hire.
Preferred Qualifications
Experience delivering homeless services in a municipal government setting is highly desirable.
Bilingual skills: ability to speak, translate, and write English and Spanish is preferred.
Peer Specialist certification or comparable lived experience credential.
Experience using HMIS and administration of Coordinated Entry assessment tools.
Operational understanding of Trauma Informed Care, Motivational Interviewing, Housing First, and state and federal entitlement systems.
SOAR Certification is highly desirable.
