Collaborate with the interdisciplinary team to promote and protect patient/resident rights and well‑being. This includes educating staff on the role of social services. Participate in the development of a written, interdisciplinary plan of care for each patient/resident to address social service needs. Hold meetings with patients/residents, families, and significant others as requested to provide support and information to improve quality of life. Perform patient/resident discharge planning activities such as family/patient education, nursing center documentation and make referrals to appropriate agencies and suppliers. Establish relationships and maintain contact and referral flow with community‑based agencies/services that benefit our patient/resident population. Qualifications
Bachelor’s degree in social work or human services field required. Must possess any certifications/licensures as required by State of employment to practice in long‑term care. Benefits
