About the Position The work you'll do is more than just a job. Consider joining our team to provide social work services for applicants and residents of the Minnesota Veterans Home-Bemidji by providing assessments, clinical support, and analysis to ensure residents' needs are addressed, and by planning and implementing social service programs for residents. This position supports the Admissions Coordinator in determining appropriate placement and collaborates with the Home Administrator to ensure delivery of services meets regulatory requirements and agency standards. The position is part‑time, primarily day shift, with at least 64 hours per two‑week pay period.
Responsibilities Provide social work services for applicants and residents of the Minnesota Veterans Home-Bemidji by conducting assessments, providing clinical support, analyzing resident needs, and planning/implementing social service programs. Support the Admissions Coordinator in determining appropriate application placement and collaborate with the Home Administrator to ensure the delivery of social services meets regulatory requirements and agency standards. Work primarily day shift, at least 64 hours per two‑week pay period.
Minimum Qualifications
Current licensure in Minnesota as a Licensed Social Worker (LSW) or ability to obtain licensure by date of hire.
One year of Social Work experience.
Skills in completing social work assessments to evaluate clients' social service needs.
Human relations skills sufficient to develop and maintain effective working relationships with other staff, residents, resident representatives, and community groups.
Preferred Qualifications
Experience coordinating placements in senior housing or long‑term care services.
Experience in a long‑term care or skilled nursing care facility.
Experience working with Veterans.
Knowledge of federal and state regulations related to skilled nursing facilities.
Experience in electronic charting systems, such as PointClickCare.
Additional Requirements
A Minnesota Department of Human Services (DHS) background check may be conducted on the top finalist(s) for the position. When conducted, a finalist must pass the DHS background check.
A Tuberculosis (Mantoux) screening (or chest x‑ray) is required prior to employment.
Employment reference checks will be conducted on all finalists. This may include a review of documentation related to job performance and contact with the applicant's former employer(s).
