MDS Coordinator – Broad River Rehab Location: Stokesdale, NC. Full‑time day shift, Monday–Friday. No medical cart responsibilities, no weekends, and no on‑call duties.

A minimum of one year of MDS experience is required. The ideal candidate has clinical experience in a skilled nursing facility and a deep knowledge of the Medicare PPS, Medicaid reimbursement, and PDPM frameworks.

Responsibilities

Assures completion of the RAI Process from the MDS through the interdisciplinary completion of the plan of care.

Initiates and monitors RAI process tracking, discharge/reentry, and Medicaid tracking forms through the EMR system.

Follows up with staff to assure compliance with documentation standards.

Completes patient assessments, data collection, and staff interviews to ensure quality practice as instructed in the RAI Manual and company policies.

Facilitates accurate determination of the Assessment Reference Date to capture resources for appropriate Medicare/Medicaid payment.

Provides interdisciplinary scheduling for all MDS assessments and care plans as required by OBRA and PPS.

Ensures necessary signatures are obtained.

Coordinates scheduling and notice of patient care‑planning conferences and communicates outcomes to staff, patients, and responsible parties.

Assists in maintaining documentation to support care delivered as outlined in the care plan.

Works with the Executive Director/Director of Health Services to ensure a care plan is initiated on every patient upon admission.

Participates in daily stand‑up meetings to communicate needs for changes in PPS timelines and Assessment Reference Dates.

Relays information from Case‑Mix Specialist audits.

Serves as a resource person for computer issues related to the MDS process.

Ensures appropriate Medicare coverage through regular communications with consultants.

Attends Medicare or Utilization Review meetings as required.

Submits final MDS to state and federal agencies and maintains validation records.

Assigns, assists, and instructs staff in the RAI Process, Medicare PPS, Medicaid Case‑Mix, and clinical computer system.

Education

Graduate of an approved nursing program and licensed in the state of practice.

Experience

Excellent knowledge of case‑mix, Medicare PPS process, Medicaid reimbursement, and PDPM.

Understanding of quality indicators and quality measures.

Knowledge of OBRA regulations and the Minimum Data Set (MDS).

Knowledge of the care‑planning process.
