Location: Sauk Rapids, MN.
Salary: $66,000 to $75,728 annually
plus a $7,500 sign‑on bonus .
Responsibilities
Perform the 12 core functions.
Apply the Minnesota Matrix Six Dimension framework.
Demonstrate cultural sensitivity.
Assist clients with payer mechanisms and obtain funding authorizations.
Attend staff meetings.
Support implementation under supervision (no client admissions, transfers, or discharges).
Requirements
Current Minnesota LADC licensure.
Ability to pass DHS background checks or obtain exemptions.
Must be able to commute or relocate to Sauk Rapids.
