Responsibilities
Provide social work services to facilitate solutions to medically related psychosocial and environmental problems of patients and families.
Ensure continuity of care by implementing interventions, including initial assessment, identification of mental health disorders, brief behavioral health interventions, information and referral during hospitalization and/or outpatient treatment.
Offer educational support to staff.
Maintain positive working relationships with vendors and external agencies to access services for patients.
Clinically assess and treat emotional mental health issues, under the direct supervision of an LCSW social worker.
Assess data reflective of patient status and interpret information relative to the patient’s age‑specific needs.
Qualifications
Master’s degree in Social Work.
One year of current experience in a clinic, hospital, medical setting, or healthcare field (preferred).
Knowledge of adjustment to medical diagnosis and chronic disease.
Strong communication skills to interact with patients, family members/caregivers, providers, and allied health staff.
Psychosocial diagnostic skills and ability to coordinate transition of care.
Active license through Arizona State Board of Behavioral Health Service Examiners; Licensed Master Social Worker (LMSW) required.
Exemption Status Exempt
Compensation Detail $68,099 - $102,232 per year (education, experience and tenure may be considered along with internal equity when job offers are extended)

Schedule Details Monday - Friday, 8:00 a.m. - 5:00 p.m.
Weekend Schedule Every 4th weekend (Saturday/Sunday)
