Seeking a Clinical Housing Specialist to join our interdisciplinary clinical team working with individuals diagnosed with a Serious Mental Illness (SMI) or General Mental Health diagnosis under Court Ordered Treatment at our Oak Health Center.

Job Summary Under supervision of Program Management, performs duties in support of therapeutic services and provides comprehensive direct support services which are structured to meet the needs of an adult population composed of SMI, dual diagnosed and/or referred persons. Responsible for working with the member to facilitate access to housing opportunities, address housing permanency and support the independent living in a community setting; performs related duties as required.

Responsibilities

Assess client needs and provide tailored housing resources.

Accompanies members on occasion to housing briefings and/or to explore housing options.

Tracks applications and referrals to housing programs and services for members.

Maintains accurate and up-to-date data demographics related to housing and homeless status.

Collaborate with community organizations to connect clients with social services that enhance their stability.

Utilize motivational interviewing techniques to empower clients in their housing journey.

Stay informed about changes in housing policies and advocate for client rights within the community.

Qualifications Requirements

A High School Diploma/GED and three years behavioral health experience; OR

An associate's degree and two years behavioral health work experience; OR

Bachelor's degree in non-behavioral health related field one-year behavioral health work experience; OR

Bachelor's degree in behavioral health related field

Computer Skills: Microsoft Word, Excel, Outlook, electronic medical record, NextGen preferred

Minimum typing speed of 25 wpm

Must have valid Arizona driver’s license, be 21 years of age with minimum 4 years driving experience, and meet requirements of Terros Health’s driving policy

Must have a valid Arizona Fingerprint Clearance card (Level 1) or apply for an Arizona Fingerprint Clearance card within 7 working days of employment

Interpersonal skills which allow for professional and positive relationships with co-workers, members, families, and other community members

Good communication skills, evidenced by a non-judgmental approach, effective listening and positive interactions

Basic understanding of the DSM-V preferred

Must pass background check, TB test and other pre-employment screening
