is looking for a licensed Rhode Island Social Worker (LICSW) to join our interdisciplinary team dedicated to the care of seniors with complex health needs. The role is fully remote and requires current licensure in Rhode Island.

Essential Duties and Responsibilities
Support the behavioral health needs of patients on an assigned caseload using evidence‑based models of care for depression, anxiety, coping with loss, mood management and substance‑use disorders.
Provide brief evidence‑based behavioral health therapies and crisis intervention as a behavioral health expert within an integrated health care model.
Consult with group residences (such as assisted living facilities) to recognize changes in patients’ behavioral health functioning and recommend appropriate interventions.
Collaborate with prescribing providers to manage psychotropic medication, monitor efficacy and side effects, and coordinate with the internal psychiatrist and clinical care teams.
Participate in caseload reviews and clinical team meetings; facilitate effective communication across the team verbally and in writing.
Complete and document assessments in a timely manner, ensuring that patient encounters are locked within 72 hours of the encounter date.
Continuously refine treatment plans for patients not demonstrating improvement by consulting with the primary care provider and using measurement‑based care.
Serve as a subject matter expert in a particular area of behavioral health care and share knowledge and resources with the broader behavioral health team.
Deliver care honoring patients’ diverse values, beliefs, backgrounds, social, cultural, and linguistic needs.

Perform other duties as assigned.
Qualifications
Master’s degree in Social Work.
Current Rhode Island LICSW required.
2+ years of prior health care / behavioral health experience.
Strong problem‑solving and organizational skills.
Experience in geriatric care highly desired.
Ability to manage multiple clients with diverse needs.
Excellent written and verbal communication skills.
Ability to perform multiple concurrent tasks with minimal supervision and meet deadlines.
Proficient in Microsoft Office Suite (Outlook, Excel, PowerPoint, Word) and EMR experience.
Reliable internet connectivity required.
High level of professionalism and demonstrated ethical conduct.
Compensation Base Salary/Wage Range : $85,000 - $95,000 plus annual bonus. Compensation is commensurate with qualifications and may fall outside the range shown.
