Social Worker Responsibilities
Identify residents’ psychosocial, mental, and emotional needs
Complete social histories and psychosocial assessments
Collaborate with the interdisciplinary team to protect and promote residents’ rights
Prevent and address resident abuse per law and professional standards
Qualifications
LTC/Subacute experience required
LSW and/or CSW required
