You’ll assist with daily living activities, observe and report patient conditions, and ensure a safe and supportive environment for patients and families.**Key Responsibilities:*** Perform personal care needs promptly and respectfully* Assist with bathing, dressing, grooming, and feeding* Take and record vital signs as directed* Provide skin care and prevent tissue breakdown* Assist with ambulation and prescribed exercises* Maintain cleanliness and order in the patient’s living area* Offer respite care for families and caregivers* Document care provided and observations accurately**Qualifications:*** Active **Home Health Aide Certification** through the California Department of Public Health* Understanding of hospice philosophy and comfort with end-of-life care* Ability to read and follow written instructions and document care* Empathy for the needs of the ill, injured, and frail* Current CPR certification* Valid driver’s license and insured, reliable vehicle**About Seaport Hospice**Seaport Hospice is part of the **Pennant Group**, a nationwide network with over 300 locations.
