Home Care Scheduler at WelbeHealth WelbeHealth’s PACE program empowers seniors to remain in their homes by offering comprehensive medical care and community-based services. As a Home Care Scheduler, you’ll play a critical role in ensuring that our participants receive the highest level of care.

Responsibilities

Coordinate scheduling for Home Care Assistant / Personal Care Assistant assignments as directed by Home Health RNs and the Home Care Supervisor, including schedule changes and communicating with all stakeholders.

Complete required authorizations in the electronic medical record (EMR) system for approved personal care and home health services.

Ensure all medical records for clinicians, home care aides, vendors, and in‑home staff visits are received and uploaded into the electronic health record (EHR).

Consistently implement and improve procedures—such as making reminder calls, clarifying instructions, or sending necessary materials—to ensure a successful experience for providers, staff, and participants.

Maintain current, accurate records of all scheduled home health appointments and home and personal care assignments, detailing scheduled times and outcomes.

Qualifications

High school diploma or equivalent.

Minimum of one year of experience as a scheduler in a home health or home care agency.

Preferred: Minimum of one year experience with an EMR and scheduling system.

Compensation & Benefits Salary: $22.12 – $29.21 per hour, plus bonus and equity.
