Position Description The Day Hab Community Connections Coordinator (DCCC) is part of TNC’s Community Connections Team, ensuring all daily activities are planned and carried out to support the safety, health, and personal interests of the individuals in TNC Community’s care. The DCCC provides day habilitation services to adults with developmental disabilities, including classroom activities in exercise, fitness, crafts, leisure and daily living skills, as well as feeding and personal hygiene assistance. Opportunities to assist with community off‑site activities also exist. DCCCs advocate for the removal of barriers for individuals receiving services through TNC Community and are mandatory reporters of all suspected or alleged abuse and neglect in accordance with TNC policies.

I. Program Responsibility / Essential Functions

Identify activities for each individual that align with the interests and goals outlined in their Individualized Support Plan, including classes, hobbies, and opportunities to try new things to broaden social networks.

Encourage enthusiasm and facilitate interpersonal connections with others.

Obtain consent, ensure accessibility, register, schedule, and support individuals throughout enrichment activities, often providing side‑by‑side assistance.

Assist with grooming needs such as tooth brushing, hand washing, and feeding/lunch cleanup, promoting independence.

Support individuals in reaching their ISP goals through classroom or outing participation.

Determine staffing requirements with leadership to maintain appropriate staff‑to‑individual ratios.

Maintain and post accurate activity calendars for individuals and staff.

Facilitate the program schedule and routine.

Follow supervisory directions and use appropriate behavior management techniques.

Document activities in case notes, weekly or monthly summaries, and other required records; submit receipts for spending from individual and organizational funds.

Collaborate with Development Director, Marketing Manager, and Community Connections Specialist to share stories via social media, website, and other communications while respecting privacy and dignity.

Apply behavior modification principles that reinforce desirable behaviors without punishment or abusive language.

Serve as a positive role model encouraging independence and awareness.

Submit medical records into the electronic documentation system (Therap) and update notes daily.

Communicate regularly with Community Connections Program Manager, COO, and Development Director regarding satisfaction with activities.

Request and maintain equipment and supplies needed for the Day Habilitation Program.

Maintain confidentiality and uphold individuals’ rights, avoiding discussion of identifiable information in public settings.

II. Health and Safety Responsibility / Essential Functions

Provide TNC transportation for individuals to and from scheduled activities.

Familiarize with medical, medication, and insurance information; ensure medications are administered as prescribed when on outings.

Report identified health issues to the Director of Health Services and Community RN; monitor as directed.

Document health issues using a T‑Log in Therap after each contact; for life‑threatening emergencies call 9‑1‑1 first.

III. Personnel Responsibility / Essential Functions

DCCC work schedule is Monday‑Friday, 7:30 am‑4:00 pm.

Respond to activity‑related issues at assigned sites.

Provide on‑call support for activities as needed across the agency.

No supervisory responsibilities.

IV. Financial Responsibility / Essential Functions

Complete mileage/transportation logs and submit to the CFO by month‑end.

Submit receipts for gas and other purchases to the CFO on a monthly basis.

Reimburse reasonable activity expenses after receipt submission.

Submit activity‑related bills or invoices to the CFO in a timely manner.

V. Facility / Transportation Responsibility / Essential Functions

Use assigned DCCC vans first; use other transportation as required.

Maintain TNC Community vehicles clean, organized, and safe; leave with at least half a tank of fuel after each use.

Report vehicle service needs by completing a repair form immediately after use.

VI. Other Responsibilities

Attend regularly scheduled Community Connections Team meetings, all‑staff meetings, and in‑service training/education.

Complete and file forms, paperwork, and reports according to guidelines promptly.

Assist in maintaining a safe and hygienic environment for individuals and staff.

Perform other related duties as assigned.

VII. Minimum Education and Experience Qualifications

Must be at least 18 years of age.

High School Diploma or GED required.

Minimum 2 years of experience in social services.

VIII. Preferred Education and Experience Qualifications

Associate degree or higher, or equivalent experience.

CNA or CMT certification.

Experience working with IDD individuals in a professional context.

Experience planning or facilitating community activities.

IX. Communications The position requires proficiency in English, with sufficient written, verbal, and nonverbal communication skills to engage effectively with individuals, staff, volunteers, community members, and administration.

X. Additional Requirements

Clear background checks and drug/alcohol screenings.

Current CPR/First Aid certification.

Maintain current Abuse/Neglect Training.

Completion of Gentle Teaching or equivalent training.

Participate in all mandatory trainings and re‑certifications.

Ability to drive motorized vehicles.

Valid Class E Missouri driver’s license or equivalent.

Own transportation with proof of insurance (may be required).

Ability to lift 75 pounds and move items or persons, including using lift devices or two‑person lifts.

Flexible schedule.

Computer proficiency.

Other duties as assigned.

XI. Physical Expectations These physical demands are representative of the physical requirements necessary for the Day Hab Community Connections Coordinator position. Reasonable accommodations can be made unless undue hardship results. Employees are often required to talk, listen, sit, use their hands, type, file, stand, walk, reach, climb, balance, stoop, kneel, crouch, or crawl. They may lift or move items and/or persons up to 75 pounds, using a lift device or two‑person lift/transfer. Vision requirements include close vision.
