What You’ll Do As a

Transition Coordinator

within BHN’s ACCS program, you will play a key role in ensuring operational excellence and service quality across multiple integrated teams. The Transition Coordinator is responsible for the service provision of the individuals that receive services in shared living residences and their own apartments/houses in the community through case management, coordinating services, and ISP goals and objectives.

Provides direct services that ensure health and safety, support growth, and promote independence of individuals served.

Provides all intake tasks for individuals served enrolled with ACCS or transitioning from a hospital.

Provides indirect services that document and support the provision of direct services to individuals served.

Completes individually assigned program-specific responsibilities.

Completes administrative duties to ensure individuals’ health, safety, and well-being.

Attends in-services, meetings and trainings as assigned and scheduled, keeps mandatory trainings up-to-date and monitors employee training completion.

Other duties as assigned by supervisor.

Who You Are

High School Diploma or GED plus 3 years relevant experience required OR Associate’s Degree or equivalent required.

Bachelor’s Degree preferred.

Minimum of two years experience in human services required.

Valid driver’s license and a reliable vehicle available for all working hours required.

Commitment to individual empowerment and growth, human rights, and community integration.

Good organizational skills.

Excellent interpersonal skills.

Must have familiarity with internet, word processing, electronic file management, and email systems.

Excellent verbal and written communication skills.
