$22.00/hr - $29.00/hr Job Summary
The Wraparound Facilitator works full‑time out of our Yuba City office and facilitates a family‑centered, strength‑based, culturally sensitive planning process for children, youth, and families. The position includes Intensive Care Coordination (ICC) and supports the provision of mental health services. Essential Duties and Responsibilities
Facilitate and participate in Katie A. Child and Family Team (CFT) meetings. Provide Intensive Care Coordination (ICC) that facilitates assessment, care planning, and coordination of services, including urgent services. Ensure the provision of Intensive Home‑Based Services (IHBS). Incorporate Strengthening Families Approach, the Five Protective Factors Framework, Trauma‑Informed Service Delivery, Nurtured Heart Approach, and Safety‑Organized Practice throughout the Wraparound process. Ensure clients receive access to mental health services, including assessment and individualized treatment that creates a coherent, all‑inclusive approach to service planning and delivery. Education and Experience
A Bachelor’s degree in health, human services, or a related field from an accredited college or university. At least three (3) years of experience working with children, adolescents, and families in an educational or child‑development setting. Bilingual experience is a bonus but not required. Possession of a valid State of California Driver License in good standing. Benefits
