Base pay range $21.50/hr - $25.50/hr
Job Summary Provides a primary focus on daily residents/patient safety. Understands the expectations of daily nursing care in accordance with current federal, state, and local standards, guidelines and regulations, facility policies and as may be directed by supervising nurses to ensure that the efforts are implemented to maintain resident/patient safety at all times.
Qualifications
Must exhibit a positive, enthusiastic, and collaborative approach as evidenced by peer and/or supervisor support for the position.
Must exhibit the ability to work harmoniously to engage others through a supportive approach and willingness to assist others.
Successful completion of a nursing aide course in an accredited nurse training program as evidenced by a diploma, certificate, or other written verification from the school and successful completion of the Department approved nursing assistant competency evaluation or successful completion of a United States military training program that includes the content of the Basic Nursing Assistant Training Program.
Be at least 16 years of age, of temperate habits and good moral character, honest, reliable, and trustworthy. Must possess a high school diploma.
Provide evidence of previous employment.
Must possess current CPR certificate or become certified within two weeks of employment.
Must be able to read, write, and speak the English language in an understandable manner.
Must possess the ability to make independent decisions when circumstances warrant such action.
Physical Requirements
Must be able to move throughout the workday.
Must be able to cope with the mental and emotional stress of the position.
Must possess sight/hearing senses or use prosthetics that will enable these senses to function adequately so that the requirements of the position can be fully met.
Must function independently, have flexibility and personal integrity.
Must be in good general health and demonstrate emotional stability.
Must be able to relate to and work with ill, disabled, young, emotionally upset, and at times hostile people.
May be required to lift, push, pull, and move equipment, supplies, etc., in excess of fifty (50) pounds throughout the day.
Is subject to exposure to infectious waste, diseases, conditions, etc. including exposure to the COVID, (HIV) AIDS and Hepatitis B viruses.
Must be able to wear Personal Protective Equipment as required to protect against the transmission of disease and viruses.
Must be willing to be flexible with scheduled days and hours, in order to assess the effectiveness of the program on some weekend days and on all shifts.
Essential Functions
Makes rounds throughout the day to ensure that all visual fall interventions are in place.
Ensures that all nursing procedures related to resident/patient safety are followed in accordance with established policies.
Observes and assists with ensuring that personal items within reach, including call light, telephone, remote control, drink, snacks, and placement of bedside table.
Inquires about resident/patient comfort/pain and reports concerns to the nurse.
Coordinates to manage resident/patient personal needs (toileting).
Observes and assists with ensuring resident/patient safety positioning (not on the edge of the bed or chair, not leaning forward in the chair; ensures positioning devices are in place).
Maintains a record of results of rounds (QA Tools) and provides restorative nurse with a copy daily.
Reports trends/patterns of safety non‑compliance to the restorative nurse and nurse on duty daily.
Reports needs for safety equipment/supplies to restorative nurse daily.
Ensures that newly ordered safety equipment/supplies are implemented upon delivery.
Reports potentially unsafe equipment to the nurse on duty and to the restorative nurse immediately.
Assists with the distribution of clinical alerts to other staff working with certain focused residents/patients.
Answers call lights promptly.
Assists in the updating of the fall prevention intervention log.
Meets with new admissions and reviews primary safety measures.
Reports any changes in resident/patient condition to the nurse on duty.
Secures incident/accident reporting from the restorative nurse and/or nurses upon arrival for duty.
Checks to ensure that new interventions are promptly implemented post‑occurrence.
Checks resident/patient Transfer Assistance Cards to ensure they are posted, accurate and moved with room changes.
Communicates trends/patterns and individual resident/patient safety concerns to the IDT through Stand‑Up Meeting (day and evening shifts) and during early mornings for night shift.
Completes monthly and PRN education on resident/patient safety based on trends/patterns with staff.
Communicates with the CNAs to assists with determining the root cause or new behaviors associated with incidents/accidents, and potential new interventions based on the root cause. Communicates this information to the restorative nurse.
Meets with restorative nurse and corporate to review observations and recommendations for the incident/accident prevention program.
Attends and actively participates in resident/patient care plan conferences as directed by the restorative nurse.
Attends and participates in facility Incident/Accident‑related meetings.
Attends educational sessions related to resident/patient safety and shares educational information with the IDT.
Participates in Onboarding/Orientation to provide information about resident/patient safety.
Wears transfer/gait belt as part of the uniform.
Performs other related duties as assigned.
