Peer Support Specialist-BHPP
Community Bridges, Inc. (CBI) is an integrated behavioral healthcare agency offering a variety of programs throughout Arizona. We are seeking a Peer Support Specialist – Behavioral Health Peer Provider (BHPP) to support individuals transitioning through crisis services.
Job Summary The PSS provides experience, strength, and hope to individuals and their families, minimizing the emotional impact of treatment and increasing the likelihood of recovery. The PSS assists in transitioning patients to the most appropriate level of care, coordinates care with all parties involved, and ensures family and other supports are engaged in discharge planning. The PSS works as part of a multi‑disciplinary team consisting of a Registered Nurse (RN), Patient Care Technician (PCT), Behavioral Health Clinician (BHC), and supervisory/administrative staff.
Responsibilities
Provide peer support to patients and families.
Assist in transition to appropriate care level.
Coordinate care with the treatment team and involved parties.
Ensure family and social supports are part of discharge planning.
Skills & Requirements
Highschool diploma or GED required. Associate Degree or higher preferred.
Minimum of six months of recovery from substance use and/or mental health disorders required.
Health‑care related experience preferred but not required.
Valid identification required.
Possesses a Certified Peer Support Specialist certification (for screening and assessment programs only).
