We are looking for a Case Management Assistant to support patient discharge planning and facilitate post‑acute placement at Southern Hills Hospital and Medical Center, part of HCA Healthcare.
Hourly Wage Estimate:
$19.65 – $27.50 / hour. The estimate represents the typical wage range for candidates hired; the actual salary will depend on skills, experience, and comparison to other employees.

Job Responsibilities
Assist under the direction of the RN case manager or social worker with facilitating patient discharges requiring post‑acute placement or services, and identify/report barriers to discharges.
Provide administrative support in case management functions.
Qualifications
Associate's degree preferred.
Medical knowledge and terminology required.
Minimum of one year experience in a healthcare setting required.
Healthcare professional (CNA, EMT) preferred.
